Consolidated Regulation Amendments

The consolidated regulation amendments consist of:

• (1) 201 KAR 29:010 ("Activities under limited mandatory certification") with MIR - Initial Application for Limited License, and MIR - Initial Application for Licensure
• (2) 201 KAR 29:015 ("Fees") with MIR - Reactivation Application, MIR - Reinstatement Application, and MIR - Renewal Application
